Can an iron waste pipe be repainted or does it need to be replaced?
Question: Our iron waste pipe from the upstairs bathroom looks very shabby. Can it be repainted or replaced with a plastic one?

If it’s just a case of it looking tired and faded and the paint is flaking off and it’s solid then I would clean it down, prime any bare metal then use a good quality exterior paint. I like using the Dulux metal shield and it can be mixed to a colour to suit.

Yes if it is flaking, just clean and repaint. You can use different ways to do that from heat to sanding as iron pipes will handle it. But if it is damaged then there are only two ways. 1) This is more expensive and involves putting rubber infill in. This is the only advice when it's original and you are unable to get the matching old look. 2) If the look is no issue, then a cheap plastic replacement.

Cast iron waste pipes (soil pipes) have a history of lasting for years, often hundreds of years! A few key points to consider.
1. is it leaking?
2. is it rusty?
3. is the paint faded and dull?
If it's leaking a good plumber will be required. There are plenty available on Checkatrade who can repair the leak.
If it's rusty and/or the paint is faded and weathered, then lots of preparation to the rust and treat it to prevent future rusting.
If the paint is dull, flat or faded by the elements, then wash down with sugar soap wipes and rinse with clean water, allow to fully dry, then lightly sand with very fine Abranet abrasives, dust off and paint with your chosen finish. Gloss paint was the go-to finish, however, Satin and Matt are now also available. My recommendation is to choose a trade quality paint. If you're not comfortable doing it yourself (DIY), then choose a Checkatrade member that does exterior painting.
